Birmingham OwnHealth
'Birmingham OwnHealth' - is a healthcare service for people with long term conditions. It provides one-to-one advice and support to members, working with a designated Care Manager over the telephone, to help manage their condition.
If you are aged over 18, can use a telephone, are registered with us and have one or more of the following conditions you may be able to become a member of the Birmingham OwnHealth service:
- Diabetes
- Coronary Heart Disease
- Heart Failure
- Chronic obstructive pulmonary disorder (COPD), such as chronic bronchitis or emphysema
- Stroke or TIA ("mini-stroke")
- Chronic Kidney Disease
- High blood pressure
- Over 65 and would like help with a range of health conditions
