Patients living within our Practice Boundary may apply to join the list by completing a signed application form, which can be obtained from the practice or downloaded below. Please note, you will be required to provide 2 separate forms of identification prior to registration, in order to confirm your eligibility to register within Practice Boundaries. Acceptable forms of identification are detailed on our application form. (Exceptions apply)
Non-urgent advice: Everyone is welcome at Hawthorns Surgery
You do not need to provide proof of ID, address, immigration status or an NHS number in order to receive care or see a GP, if you don’t have one.
